CASE OF VAGINISMS CAN BE SUCCESSFULLY TREATED

                                  35 years old Female came with husband in OPD with complaints off  difficulty having intercourse and inability to have intercourse due to vaginal contraction on attempt to penetrate. They were married since 6 years. But all attempts at intercourse were failed. The marriage was unconsummated for 6 years. They had consulted many GP’S and Gynecologists. They consulted test tube baby center in near by metro city  where serial  sonography for ovulation etc done without  considering vaginisms. finally they consulted me (Sexologist) that too with fear in their mind.
                                      After taking complete history it was found that they have good interpersonal relationship, good attitude towards sexual life.  She use to enjoy foreplay but as soon as husband use to try to penetrate she use to feel fear and husband was unable to penetrate.  An attempt to vaginal examination was also failed due to vaginal contractions. Patient was counseled in details.  Patient and husband were educated about the illness, its course and treatment. Couple therapy was advised and medications were given and asked to follow up after five day for sex therapy. 
                 Within 15 days after 2 -3 settings of Sexual therapy patient felt great improvement and they finally had successful intercourse after 6 years of marriage. Couple was given two options either to go for a baby or to enjoy sexual life for 6 months or so and then go for baby

SUMMARY 
                 Vaginism is a conditioned Response which probably results from the association of pain or fear, with attempts at, or even fantasies of vaginal penetration. It is missed by many due to improper history. As it usually presents with infertility (due to unconsummated marriage). When Vaginismus is not treated it may persist indefinitely and deprive the couple of the opportunity to have intercourse and children. The likely hood of a successful outcome through treatment is very high indeed particularly in happily married couples. Vaginismus is a condition in women (perhaps not to often seen because it is not looked for) of both psycho and soma. Both needs to be tackled simultaneously. Medical treatment is obviously indicated for those involving organic factors but behavioral therapy and a psychosomatic approach is the most important thing as it is response acquired by classical conditioning in which the stimulus is real or imaginary.  Detailed Meticulous history, proper psychosexual education, couple therapy , behavioral therapy systemic desensitization gives good result with 100% success result. Vaginistic response can be extinguished in 7-5 days using 4 to 8 session and penile penetration can be achieved in 3- 14 weeks.
Not only vaginismus but there are many sexual disorder which can be treated easily and immediately but patients dont consult sexologist because of fear or stigma or sometimes they consult someone who is not trained in sexual medicine. So one should remember to consult as soon as possible when sexual problem arises, consult a right doctor ( sexologist ) who is trained in sexual medicine so that the married life ans sexual life can be made happy and beautiful.
